for Medical Report |

1.
|
In order to complete
all admission procedures, new students should submit
their physical check-up report to the Registry before the 1st of September, in order to prove
their physical state.
|

The physical check-up procedures are as follow:
| 2.1 |
Fill in the
student no. (e.g. p-xx-xxxx-x) on the upper right
corner of the form.

| 2.2 |
Take a fr+ee chest x-ray
test and obtain an x-ray report at the Health
Care Bureau’s TB Prevention Center (CTB)
at Estrada da Victoria, no. 40, Macao (near the
tunnel, the direction of Rua da Fonte da Inveja,
behind the traffic police station)

To complete the lung x-ray
procedure, please go to the Health Care Bureau
TB Prevention Center CTB to make a lung x-ray
appointment. When the chest x-ray report is out,
please bring it along to the Health Care Bureau
of your district to make an appointment for your
body check-up. (Students who are Macao residents
can also go to private clinics registered under
the Macao Health Care Bureau for their physical
check-up)

Please submit your signed physical check-up report, together
with a record of Tetanus vaccination (both original and
copy), to the Registry. |
4.
|
Before going for your physical check-ups, please note
the followings:
| 4.1 |
The following
documents are required:
a. I.D card, passport or visa
b. A recent photo
c. The school’s physical check-up form
d. Tetanus vaccination record

| 4.2 |
Students wearing contact
lens should change to regular glasses before
the check-up.
|
| 4.3 |
The urinary test cannot
be performed during one’s menstruation.
One can only take the urinary test after the
menstruation period.

| 4.4 |
The x-ray check-up cannot
be performed during pregnancy.
|

| Note: |
The physical check-up
takes time to process, and hence, you are advised to
get it done as soon as possible. For any student who
fails to submit the physical check-up report, MPI reserves
the right to cancel his or her admission eligibility.
